Home Health Link is a program funded by Queensland Health for the Gold Coast Health Service District. It is available to provide short term support and assistance for people in their own home.
The aim of the program is to avoid preventable visits or admission to hospital, or to assist patients to leave hospital earlier than would otherwise have been possible in a safe and supportive environment.

• Any person who has been assessed as being at risk of an admission to a Gold Coast Public Hospital
• Any person at risk of visiting the emergency department of a Gold Coast Public Hospital.
• Any inpatient of a Gold Coast Public Hospital who can be safely discharged home earlier if supports are made available.
• Any person who can reduce the likelihood of re-admission to a Gold Coast Public Hospital if supports are made available.


Home Health Link will work closely with doctors, hospital staff, patients and their families to ensure that clients are provided with the services required to ensure that they can safely be at home.

Services can include;
• Nursing
• Allied Health - such as Occupational Therapy or Physiotherapy
• Personal care - such as assistance with eating, bathing or dressing
• Domestic assistance
• Supported Transport - for discharge from hospital or emergency department, or for visits to Doctor or other health services
• Carer support and respite
• Child care
• Medical equipment - short term loan as a part of a care package
• Assistance with Meal preparation
• Emergency Accommodation
• Overnight Support
