The goal of the HSS Case Management System is to:

• Give doctors a high level of control over the care of their patients
  
• Avoid hospitalisation or decrease patients length of stay in hospital
  
• Decrease rates of hospital re-admission from a patient’s primary or secondary diagnosis
  
• Shorten patient recovery time
• Refer patient to community resources as appropriate

How To Refer

HSS services have no waiting periods and if necessary the patient can be seen the same day as referral.

Select HSS as your preferred provider when entering details via Medical Objects or Argus

E-Referral

Referrals can be made by phoning (08) 8372 4999. A Case Manager will take the referral over the phone.

Call 1300 550 654 and an experienced Case Manager will discuss your patients needs and complete a referral over the phone.
